Descriptive History:
The Cornell Medical Index Health Questionnaire (Brodman et al, 1949,
1960) is administered on every fourth visit. The subject responds to
195 questions about his/her symptoms, health habits and family
history. A separate questionnaire is administered to women and men.
Data collection began in 1960 and has continued to the present.
